This site is for authorized personnel to schedule COVID-19 tests for municipal agencies.
SCHEDULING INSTRUCTIONS
1. Enter employee information:
First and Last Name
Phone Number and Email address
Date of Birth
2. Enter Reason for Visit. In this section please enter the following information:
-Municipal agency (City of Carlsbad, City of Encinitas, Rancho Santa Fe, etc.)
-Type of test(s) requested: RT-PCR, Rapid PCR or Rapid ANTIGEN
-Any pertinent additional information or requests
Once an appointment is submitted, the employee will be sent two .pdf forms which contains information on how to get to our facility and how to access their lab result if an RT-PCR test is performed.
It may take up to 45 minutes for confirmation of an appointment once submitted. Please take this into consideration when selecting an appointment time. The employee will be sent required forms to be signed electronically which includes a COVID Questionnaire and Medical Records Release Form.
When results are available, they will be emailed to the proper personnel on file.
Rapid PCR and Rapid ANTIGEN results should be available within 2 hours
RT-PCR results typically takes between 24-48 hours.
