To expedite your check-in process, please have your insurance card and driver’s license available.  If you are a new patient, you may also download, print, and complete a Patient Intake Form to submit at your first visit. 
Prior to check in, please take the time to review our Terms of Use, Notice of Privacy Practices, and Patient Rights and Responsibilities. These forms contain information about how medical information about you may be used and disclosed, as well as your rights and responsibilities as a patient.
